Aggressive male pattern baldness or CTE?

Hey guys I’m a 25 year old male and I began aggressively losing my hair in September of 2020. About a month before the hairloss started I experienced horrible burning/itching on my scalp and all across my back everytime id sweat it was odd. Didnt think anything of it until I noticed my hands where covered in hair everytime id shampoo my hair. Next thing I and my wife noticed was my lower region hair was falling out as well as my chest hair and my beard got thin and patchy.

Over the course of the next 7 months my hairloss has dramatically slowed down after about the 4month mark although it is still visibly getting worse. My body hair seems to have recovered well but my hair has not as im still shedding between 30-50 hairs a day which is not normal as I dont recall ever shedding any hair until this. I went from a thick dense head of hair to practically bald I can see my scalp across my entire head and the only part of my head that seems to be spared is the back.

I visited a dermatologist 3 months into my hairloss and he told me it was most likely Telogen Effluvium and hes already beginning to see regrowth and a lot of it but Visually its only gotten worse. At the 6 month mark of my hairloss I visited him again and this time he took a biopsy sample from my left temple where I have ever so slight recession which Ive had since Ive been 17. Biopsy results came back as Inflammation and Mild Androgenetic Alopecia, Dr gave me finasteride and a topical corticosteroid and sent me on my way. Im just confused and I’m not sure what to do as if this was mild Androgenetic Alopecia wouldnt I not have lost over 50% of my hair in the course of 6-7months??? Ive began taking finasteride and im one month into it but at the 3 week mark my shedding has dramatically picked up again its thinned my sides and my entire top of my scalp even more. Ive attached pictures and ive just buzzed my head for the 2nd time in my life as my hair was becoming to thin to style. Anybody have any ideas what this could be its ruining my life literally.

Have you had a blood test?
Also shedding is v normal a month into finasteride, can take quite a while to see a visual difference.
Yes I’ve had a blood test everything was OK except for my prolactin was elevated. It was just slightly out of range and the Dr thought nothing of it.

Early stages of male pattern baldness. What's your family history?
Absolutely no balding in my family on either side and its just odd for it to be so aggressive. The thinning is equal along my sides and top as well isn’t that not indicative of male pattern baldness?

The shedding started by finasteride could be a good sign that it’s doing something to help your hair. If a biopsy picked up male pattern baldness, that’s what you probably have. The initial shed may have been caused by something, accelerating your male pattern baldness. You can maybe do another biopsy, this type on the side areas just to make sure. But the loss of hair in the crown and on top for the most looks like male pattern baldness

The initial shed may have been caused by something, accelerating your male pattern baldness. You can maybe do another biopsy, this type on the side areas just to make sure. But the loss of hair in the crown and on top for the most looks like male pattern baldness
Wouldnt the loss on the top more or less be in a horseshoe type pattern starting at the crown and at the temples and not equally across the entire top of the scalp? The biopsy picked up Androgenetic Alopecia as it was done right at my temple and ive read that in 90% of people mild Androgenetic Alopecia will be present even with no balding. Do you think I should continue on with finasteride? I did have an initial libido boost for the first two weeks as well as crazy morning erections like when I was a teenager which makes me nervous that its doing harm and not good.

finasteride will help mild androgenic alopecia too. In the meantime you can take it and investigate more about your hair loss. This way you can cover all angles

Might be DUPA, but it's still a form of male pattern baldness. When the top and the vertex are both diffusing in the NW6 pattern, it's definitely androgenic. You need to give finasteride at least 6 months.

Is this initial shed after just 3 weeks of finasteride normal? Im terrified of reflex androgenicity and what makes you say definitely androgenic?

That is definitely normal early on. If it continues after 3-4 months then you can begin to be suspicious of RH.

What brand Finasteride are you using? Shedding could be related to Telogen Effluvium. You using anything else besides Finasteride for the hairloss? Anti inflammatory shampoo like Ketaconazole?

Im using Regenpure Dr every other day, i might drop the finasteride and try RU. Its been a little over a month now and alls the finasteride has done is make my scalp extremely itchy and tingly and make my shedding worse. It is the generic version of finasteride. Odd thing when I observe the hairs im losing theres maybe 1 or 2 out of the 50+ hairs I lose that are actually miniaturized -_- no idea what is going on but my hair is going to sh*t very quickly

This is absolutely not true. I shed for 9+ months while my hair was improving on finasteride, and off and on for years while maintaining the exact same density.

Shedding does not instantly mean something negative and certainly does not mean RH.
