To answer your questions:

2. Probably, depending on where you bought them from.
3. 0.5mg/day is recommended for BPH and seems to be the most common dosage used for treating male pattern baldness. There is no 'official' recommended dose for hairloss. GSK planned to use 2.5mg/day for 6 months followed by 0.5mg/day in the Phase III trials.
4. It has been approved for treating BPH, and has a similar safety profile to finasteride.
