I'm using ketoconazole but it doesn't seem to be effective at all against the pimples/scabs I usually get. I use a gel for my face that contains benzoyl peroxide and it treats my acne well. Would this substance be harmful to my scalp?

I'd personally try zinc picolinate, coal tar, salicylic acid or azelaic acid on my scalp before BP, just because of its drying and sun sensitivity properties.

Thing is, I pretty much have tried everything of these you mentioned. The only one that has been working, at least a bit, is ketoconazole 2%. It alleviates the itch but doesn't prevent the pimples/acne. They don't itch but I end up hurting them while washing or combing it.

Hmm. There are other acne preventions but you've probably tried those too. Maybe give BP a go, but just make sure you wear a hat/use sunscreen and moisturise if you notice excessive dry skin.

It's too soon to call it an update, but since I created this thread my scalp is at its best. No itch, no stings, no pain, at least for a while. It's been some time since I felt this good!

I started using ketoconazole every other day as of the week I started this thread. Applied benzoyl peroxide on my face, especially forehead where I get more pimples. I also applied on some spots I had on my scalp, but only once a day at night, washing it the following morning. On Tuesday this week I started using a tea tree shampoo instead of keto, and so far no signs of itchiness. Maybe it's too soon to say I won, but I recognize that as an accomplishment since my scalp has been torturing me for months
