Looking back I think I experienced a significant frontal recession about age 18, taking me back past NW1. I initially didnt know what to think of it, however during my sophmore year of college began to freak out a little bit about my changing hairline. Mom (bless her heart) even sent a few pics of male catalogue models with high hairlines to build my confidence. I should have taken the hint and panicked, but didnt.

Thru my 20's things changed minimally, however looking back I can see a gradual change in photos (diffuse thinning). I started finasteride at around age 32. I was then blissfully unaware of my hair for about 5 years. No other treatments. Over the last year Ive had trouble maintaining an adequate hairstyle (mussed look with long bangs doesn's work) and eventually got my first buzz-cut. While I was pleased with the overall easy maintenence I was again freaked out by my hairline... A scar which had been comfortably within the hairline had migrated out beyond the edge. There was also notable diffuse thinning, frontal and vertex (NW2-3 with frontal and temporal recession and diffuse thinning). So, I found these hairloss sites and learned what there is to know (beyond finasteride).

By mid-april 2005 I had switched from proscar only to a regimen including dutasteride, minoxidil and nizoral. I am still playing with products from dr. Lee's which might add azelaic acid/retin-a to the treatments, and am considering T-sal. In any case I plan to settle in for the long battle... I am on a lot of products, but my impression of the state of the science is that no one drug has 100% efficacy (and, I am already "failing" finasteride, making my situation a little tougher). I am shooting for long term maintenence (decades if possible : ). As nobody has studied combinations of medications I am not sure that this goal is possible, but I am hopeful, and will do what I can with the tools available.

I could mention that I have always had fine blond hair which is getting finer. Without this site I might not have noticed hair shaft miniaturization (which is quite obvious when you look for it) to be a marker for male pattern baldness. I will use that as a marker for hair health and regimen efficacy.

As I am adopted I have no idea what my genetic predisposition to hair loss really is, but looking at myself over the years I believe that I am on the path to solid NW6 without intervention and need to do all I can now to stop the thinning trend.

I have hope that something useful will come of research over the next 10 years (growth stimulants using the hedgehog pathway?) and hope to hang on for those developments (lord willing, knock on wood).

Anyway, Im enjoying the site (but addicted to posting, .... hope to post less and forget about the hairline again once Im sure treatments are working) and appreciate the experience and advice I find here.

6 month update

In april 2006 I quit finasteride after 4 or 5 years and started multiple treatments...

I initially started Dutasteride .5mg, 4 pills each day = 2mg... Planning to emulate the Phase II trial for 6 months then go to .5mg. I also started to use minoxidil (dr lee's 5% to top and frontal, 12.5% at hairline) and nizoral. At times I even used spironolactone 5%, but haven't really been consistant enough with that for it to count. Oh, and I used retin-a containing minoxidil for a while. Oh, and some MSM, and multivitamins : ) Oh, and tricomin : )

To complicate matters I bought an electric shaver and have been buzz-cutting... I did no blade, but didnt like it as much, so am using tapered, #4 and #8 with better results.

Initially (after 2 months) I felt I was experiencing thickening and even had some new fuzz at the temples. After 6 months I feel my temples are slightly better than at the beginning (thicker, maybe a few new long hairs).

My crown feels and looks thicker (not a concern any longer : ) It was never truely gone, but in the right light it was becomming concerning...

My frontal hairline is the thinnest. I never noticed any fine new minoxidil hairs in the front, and cant tell if there has been any change... Some days i imagine its worsening, but in general I believe it is unchanged.

Summary: subtle overall coarsening, subtle temple improvement, some crown improvement. No definite change in the frontal area. I was panicking at the beginning of treatment but am now fairly confident I am "stable," and not worsening. That is "success."

Side-effects: I actually felt really good (stronger) the first month on high dose dutasteride, but after 2 months of 2mg/day lost all libido... I quit dutasteride for a week or two, then resumed it at a lower dose and after a few more weeks returned to normal. I am hoping to find that I tollerate between .5mg and 1mg each day without side-effects... I will attempt to adjust this to find the lowest effective/tollerated dose.

Plans: I will continue generic dutasteride .5mg one or two a day, along with minoxidil once a day and nizoral 1% shampoo... If I notice backsliding I might give the spironolactone/retin-a/higher dose dutasteride/tricomin another chance... If I have more loss of libido I might have to return to finasteride... Too soon to say.

1 year update

One year ago I switched from finasteride .5mg to dutasteride, and started experimenting with topical treatments for this first time. This was motivated by an awareness of progressive thinning despite finasteride (for years). I think that overall my hair is a little thicker (especially at the crown, which has normalized) and shafts are a little thicker, but there has been no foreward movement of the hairline... Still, this stability is a relief and I consider slight thickening a success. Things I have learned this year...

1) Finasteride doesn't work perfectly for everybody, and if it isn't working dutasteride might provide some improved benefits... I had side effects at higher doses 2.5mg/day but at 1mg/day feel fine. As with finasteride, side effects (headache initially, decreased libido) deminished and disappeared with time. I am glad to have the option of dutasteride (dutas) but it isn't for everybody.

2) Dr. Lee's minoxidil is MUCH MUCH better than the over the counter greasy crap... It dries quicker, and also comes in lotion for hairline and mixed with retin-a / azelaic acid if you believe in that. I use it once a day, so I can't swear that my slight thickening isn't due to minoxidil rather than dutas.

3) Tricomin is convenient and feels good... I like it but often forget to use it. Some slight suggestive evidence makes this a "maybe" and I will resume it if I start worrying about my hair again.

4) Nizoral Shampoo is easy to use, and might possibly have some benefit as an anti-androgen... So I use it.

5) Short hair is good... I cant have long bangs anymore, and dont have the volume I used to. A buzz cut (shorter on the sides, longer (about an inch or a little more) on top looks much better than dealing with longer but thinning hair. Shorter looks fuller.

6) Garnier "Surf Hair" paste is good stuff... In general better than heavier gells, giving texture and a fuller look.

7) spironolactone creams (even the "good ones") smell kind of bad... I will use them if and when I have to, but they are not "first line" drugs.

8 ) United Pharmacies http://www.unitedpharmacies.com/ has been reliable.

Anyway, this site has been very very helpful... I'll post updates periodically and will certainly post more if things aren't going well : )

Thanks for the update cornholio, its good to hear from other dutasteride users! your story is certianly a success and gives us dutasteride users something to look forward to...
