I suppose I'll start with a brief history: Basically, I had cancer and lost all of my hair in mid 2009. Following remission later that year, it grew back in full until a few months later I started experiencing rapid loss around my whole hairline - that is, the whole line of my hair (front, sides, back, around every edge).

The loss started in spring of 2010 and I've been losing hair ever since, however, the hair I've lost on the top of my head and to some extent the sides, has not grown back.

Over time the loss has progressed from the front of the top of my head to the back (as seen in the picture, just a couple of years ago it was completely filled in), and with the line of loss there has always been an extreme amount of pain/tingling which (I think?) precedes the loss. At any given point I can lightly tug on my hair where it formulaically should be lost next and 4+ will come out. I can do this around my ears and with the hair along my neck too.

Hairs will grow back on the top of my head but then fall out again until they get shorter and shorter - I assume this is miniaturization. The same happens in other areas around my head not typically lost in male pattern baldness, however those keep coming back and the ones on the top of my head do not.

I've been to many different doctors and have not had luck with finasteride (had rough side effects) /Rogaine, or a variety of supplements. Therefore, I'm pleading to you guys for help. Do you think this is normal male pattern baldness, a different condition that has been exacerbated my male pattern baldness, or something completely different? Needless to say, I'm desperate for any help or words of wisdom.
