difference between the first third and the vertex

Hi, I don't understand. I started and ended by being badly attacked in the first third and the vertex 6 months ago, my vertex has been completely restored as a normal person but the first third is the trunk.

There is no regrowth on the first third and the fall continues abundantly, why such a difference between the first tier and the vertex?
