Yes, I feel your fustration. I'm in No. VA. I actually went through the same thing today.

Dr. Chadab (MD) derm - nice guy but ran no tests and basically prescribed spironolactone. no tests. no scalp biopsy
Dr. Nigra (DC) derm - rogaine. No tests. No scalp biopsy no nothing

Again, these doctors were recommended but did not do any tests. It's fustrating. You're right you're basically on your own.

It's important that we stick together and help each other out. Anybody out there who has found a good derm - let us know!
