Does anybody have any thoughts on my scalp (telogen effluvium or sebum)

since i was 19 i was loosing my hair with a white bulb at the end of it. About a year and a half ago I went to the dermatologist about my hair loss. She checks my hair with a otoscope for about 10 seconds and does a pull test and tells me i have alopecia and dandruff. Then i tell her that hairs that fall have this white bulb at the end of it and its been going on since i was 19 tells me that i have telogen effluvium. then i get prescribed finasteride, ketoconazole 2% shampoo, and minoxidil 7%. Now 23 still use finasteride, ketoconazole 2% shampoo but not minoxidil 7% it irritated my scalp did use minoxidil 5% but stop after 5 months saw no improvements. About 3 month still loosing lots of hairs but do see less falling and its been going like that since then but recently i started loosing lots of hair again. I'm pretty sure I have chronic telogen effluvium (my scalp is very visible when wet and oily) but I also think I'm loosing my hair because of sebum. My hair gets oily through out the day and after I sleep and wake up I'm just covered in oil I do shower every morning. After I shower I let my hair dry and I comb it and there's just tons of hairs falling out nothing new but now its lots more. I'm posting pictures of my scalp and the white bulb if anybody has any ideas what's going on on my scalp.

Same Problem

Started 5 years ago, lost a lot ground, hair gets very oily every day.

Nothing helped so far

Could be seb derm which can actually cause temporary hair loss if you don't have the baldness gene. Try keto 2% I had this same issue when i was 19 after a year battling it my hair actually regrew. It's the inflammation that causes shedding.

Holy cow that's a lot of sebum. See if you can get keto 2% but honestly you may need to address this internally as well. You may find some benefit from dutasteride as it blocks the type I 5AR enzyme which plays a role in the sebaceous glands.
