Any medication is going to absorb better and more slowly with food. The water helps assure that the pill does not get caught in the esophagus. The esophagus responds with peristalsis to a "bolus" (ball) of food that you've chewed up and swallowed. Sometimes, pills do not do the same job of causing persistalsis due to their size. They can get "lodged" in the esophagus because of this. Water, however, does not need peristalsis and uses gravity to descend from the mouth to the stomach. If you drink a lot of water, you assure that the pill does not become stuck. Food also stimulates gastric secretions in the stomach (HCl, enzymes..) that will aid in the breakdown and absorption of the pill in an efficient manner. If there is other contents in the stomach, I also believe that it will cause the pill to absorb a little more slowly and evenly... and more efficiently when you realize that as an azosteroid, finasteride is
probably best absorbed with fatty acids.
That said, the fact is that Merck doesn't make a suggestion concerning food. They do, however, urge a full glass of water with each pill. If it's making you ill though, it's always best to try to control the digestion a bit by taking it with food. That's my personal opinion and suggestion.
The same time everyday is not necessarily as important with finasteride as it is with other drugs. HOWEVER, it doesn't hurt AND it will probably decrease the likelihood of missed doses (it ensures compliance).
