Greasy Scalp And Flakes, Can It Be male pattern baldness?

I have been experiencing hair loss with excessive shedding (over 100 strands daily, maybe up to 300 strands when I shower) for over 4 years. My scalp feels oily and I have large white/yellow flakes on my forehead. These flakes makes my scalp very itchy and I also experience some burning pain from certain spots (mostly in the forehead and temple).

Today when I looked closely I noticed that I could slowly scratch off the greasy flakes which contained hair strands in them. I attached an image of one of the flakes containing 6 hair strands in the bottom of the thread.

Is it possible that the greasy scalp cause the scalp pain and the hair loss, or is this common in male pattern baldness?

(I have already tried using Ketoconazole shampoo but it does not seem to make my problem any better.)

I have had what you have for years, I used to use coal-tar based shampoo which helped a bit but would cause huge flaking. I suggest you try a piroctone olamine based shampoo like Neutragena T-Gel Greasy which for me at least relaxed my scalp as soon as I applied it, amazing stuff. I have n't had any relapse since using it.
