Hair loss... Even when it's set to end, it is terrible

Considering a hair transplant and I'm terrified and indecisive. Will I really be OK with bald crown? Shall I opt for FUT first in an effort to maximise yield or go for FUE in an effort to minimise scarring and allow me to buzz the head? How many body grafts is too many, and how will they look anyway? Very complicated questions and nobody really knows the answer. I wish that I didn't have to worry about this for another 10 years. I really did try to live bald. Now i have saved up enough for a hair transplant and I can't even decide what would be best for me. I'm stuck in the inertia of being a bald loser.

i think mildy bald crown and good hairline can do the trick while older with short hair! or with buzz cut to do the illusion of Norwood 1. That is what i will certainly do in the near future bro.

Completely bald crown with horseshoe visible from behind. That's me. Mildly bald anything is OK but I am aggressively bald.

Completely over. And now I realize that it was over so many years ago. One of my first posts on this website I had a photo of wet hair and asked about it, and somebody noticed a NW7 pattern. It was over then. I saved up diligently for a hair transplant and yet the hair loss is so aggressive that I can never get full coverage and keep the insurance of being able to buzz to 1 all over. Seriously what is the point of putting effort into anything

Going to be brutally honest, people can’t tell I’m losing hair, they just point out my grey hair and I struggle with dating. This is without a bald crown.

You suit a buzz head so maybe having a hairline, buzzed down will help

@doubleindemnity what is your regimen again?
If you had a medicine that could give you great hair but huge hypertrichosis would you take it at taht point?
Only minoxidil. I'm scared to use finasteride or dutasteride. Micro needling never worked for me. And soon I will try this Zix stuff but I am not expecting much. Regrowth is almost impossible. Even the best treatment can only be expected to maintain. And I have nothing to maintain now. Photo was one or two weeks after no guard buzz cut.

You have a good head shape, skin tone and body. I think you'll be fine leaving the crown bald

@doubleindemnity ; how old are you bro? you ar taking oral min? which dosage and for how long? no crazy unwanted hair from it?
If i was in your case i would do a hairline transplant( if you have receded hairline), and shave head like nw0 buzzcut

I use standard topical min. I have no photo. Or, that photo from behind shows my hairline.

Would I really not look like a fool with temple points but with a horseshoe? Does it look natural? I can't use fibres or concealer on the crown ; it's too thin. That photo is how it will look.

Some people like Prince Harry and Zidane lose the crown but still have their hairline. It's rare but exists. I think you'll be fine.

Sorry for my ignorance as I am not regularly reading this place, but have you consulted a reputable transplant surgeon? I think you should see 2-3 good ones so you get an overall opinion on what is possible. From what I can see, even that horseshoe at the back is thinning another inch down, you have decent hair calibre but the advanced pattern negates all that. I agree with @michel sapin says, buzz cut transplant seems like the only viable option, in the sense you spread all the grafts all over including some in the crown and then shave it down with smp perhaps.

I don't think you should have expectation of growing it out. Because you'll still be in bald territory and the bald guy from my experiences in interacting with girls/guys and using common sense. You'll still likely have a receding hairline, diffuse thinning, bald crown and horseshoe pattern being visible, balding donor, also with a layered combover. Even one of those can screw a guy up, let alone all of them. But see a Dr first, once he sees your donor capacity and sees how many grafts there and multis, he can make a judgement. It could be the case you return with a decent head of hair, one of those rare Norwood 6s/7s we see on youtube that do. What's worse than baldness is a odd surgical head that everyone picks up on or looks off in some way so you must be extra careful which some members ended up with and a waste emotionally/financially. You're not a thick Norwood 3 going in for corner building.

Lastly, do what makes you happy though, maybe some hair would help, but remember we get only one scalp and whatever is done is usually permanent. Personally I don't think there will be much cosmetic difference from what I can see if you grow your hair out post hair transplant due to the limited donor and advanced pattern. However, a buzz cut transplant will look good with smp and you have an excellent head shape. But then you may think what's the point of that and the financial/emotional journey just to shave it down. However, some frame may help you even if you don't have hair in the conventional sense, it can often look cool, shaved through choice option.

Think it all through and best of luck. As a plastic surgeon told me, other surgeries give way more bang for buck but transplants are limited cosmetically unless it's <NW3-4 generally speaking.

Same time (even with the same surgeon) there are cases where it goes really wrong even in ideal candidates.

@justinbieberscombover may know some examples of poorish results (but not fail as such) and see how you feel if you had that. If you feel okay and it's better than now, go ahead.

I would recommend waiting on the hair transplant for a couple years to see where verteporfin ends up. The drug may be able to regenerate hair in donor region after transplant.


Even if it doesn't grow back hair, it could prevent scars and that is still a massive improvement. The biggest problem with buzzcuts after a hair transplant is the presence of scars.
