Possible telogen effluvium?

This all started 3 and a half year ago. I was going through a tough time. I just started college and was not eating healthy throughout the day. I was out in the dust and smoke all day and couple of months before my hair fall started, I fasted a month (not eating+drinking anything from morning till sunrise, I know you think I am crazy, but had to). And because I was living in a country with intense heat almost entire year, fasting was really hard on me and on 2/3 occasions in the month long fasting, I got sick and almost puked.

So, in November of 2009 I stared loosing a lot of hairs (around 300 daily). I was using Garnier shampoo prior and would lose only 2-3 strands a day during shower, so this was quite unusual for me. I switched to Head and Shoulders, which stopped the hairloss for 2/3 days, then it would start again. I would easily loose 100-150 hairs while combing and about the same while in shower. I had a full head of hair and now its very very very thin. Just about 4/5 months ago, I noticed that the hair I am loosing is much much much smaller in size than the size of my actual hair. I also noticed that there are a ton of small and very fine thin hair (possibly not vellus hair because it the hairs I am talking about is longer than vellus hair). Now when I comb, I lose about 30-40 Terminal hair, and about 90-100 of those very fine thin hair with thinner top. All these hair have the keratin balls if youre wondering. Lately, the hair loss have reduced to somewhat degree as I started going to the gym, taking vitamins and protein shakes and started eating healthy. I can not possibly live with the shame of having a near bald head. Will I ever grow back?? Nobody from my dads side is bald, my dad is 65+ and has a full head of hair. All my uncles do the same, cousins, from my dads side also have hair. My maternal grandfather was however bald and so are all my uncles, but they lost their hair in their late 30's. My mom also suffered from Telogen Effluvium in her late 30'sand never grew her hair back unfortunately. I am very worried. I am a 23 year old male, ethnically from the Indian Subcontinent. Also, I live in the US and I heard there is chlorine in water here, does that contribute to the hair loss in any way shape or form?
