"Change from baseline in scalp DHT was -70% for P-3074 o.d. and approx. -50% for P-3074 b.i.d. and the tablet. Serum DHT decreased by 60 - 70%. The doses of 100 and 200 μL P-3074 resulted in a -47/-52% scalp DHT reduction, similar to the 300 and 400 μL doses (i.e., -37/-54%). A -5.6% inhibition was observed for the vehicle. Serum DHT was reduced by only -24/-26% with 100 and 200 μL P-3074 and by -44/-48% with 300 and 400 μL P-3074. No relevant changes occurred for serum testosterone."

"At week 24, the combined solution of finasteride and minoxidil was significantly superior to minoxidil alone in improvements of hair density, hair diameter and global photographic assessment (all P < 0.05). About 90% of patients treated with the combined solution experienced moderate to marked improvement. The combined solution also had minimal effect on plasma dihydrotestosterone levels, approximately 5% reduction. There were also no systemic adverse events reported by patients in both groups."

"After multiple doses, mean (± SD) finasteride C(max) and AUC(0-t) corresponded to 0.46 ± 0.28 ng/mL and 6.64 ± 7.50 ng/mL x h for the topical solution and to 6.86 ± 1.78 ng/mL and 57.93 ± 29.38 ng/mL x h for the tablet. Plasma DHT was reduced by ~ 68 - 75% with the topical solution and by ~ 62 - 72% with the tablet. No relevant changes occurred for plasma testosterone with either treatment. No clinically significant adverse events occurred."

So some studies show it to reduce serum DHT more than oral finasteride, some show it to reduce it less. More studies on this would be great but at least we know that its effective at reducing scalp DHT and actually better than oral finasteride

Ive used both oral finasteride at 1mg and pharmacy-compunded topical finasteride at 0.1% or 1mg/ml and I'm on both rn but oral destroyed topical in terms of effectiveness for me.

yea unelss you have a super effective vehicle the least u can use is 0.25% to get something comparable to oral finasteride

your dosage was quite low

the dermatologist gave me a topical with 7% minoxidil and 0.1% finasteride. What about topical dutasteride?

I made my own finasteride in Rogaine (0.06%) and used it for two months. Huge disappointment. I need to see what the vehicles were comprised of in these studies...

I think 0.06% is so low. you should add minimum 0.1% finasteride to make a topical solution. and also they used 0.25% finasteride in the study : https://www.ncbi.nlm.nih.gov/pubmed/29972712
that means they used approx. 4 times more finasteride than yours. 0.06% is so close to placebo effect imo.

That's not true at all.

0.06% means 0.5mg per ml. Let's assume that only half of it actually made it to your scalp, that's still 0.25mg.

When you take oral finasteride, assuming standard dosage 1mg daily, your liver kills half of it even before anything enters the bloodstream. The rest of it, 0.5mg, is then distributed throughout your body tissues, with a margin of it reaching the scalp. Do the math.

And by the way, your skin, especially scalp, is a perfect site to get a chemical into your bloodstream, hence why you see things like progesterone cream, DHT cream etc. with topical use as the prefered method of administration.

DHT is a paracrine/autocrine hormone. It's made where it's used. Serum level is by far not all that matters.

Less than 1% of finasteride gets absorbed past the stratum corneum when using hydroethanolic vehicle

I will add that they measured plasma exposure in the study - I imagine that plasma exposure from a scalp topical application may not reflect the amount reaching the scalp follicles; the amount reaching the follicles may be more.
