What To Do When Finasteride Fails - 20y/o?

20y/o been on finasteride for 7 months still losing easily 100+ hairs a day - 30 on pillow, 30 in shower, 30 drying and then every time i run fingers through hair etc. I figured finasteride would have slowed it down by now as everyone else seems to react with first 3 months etc however i've had no results. thinning all over top of head.

i will continue for next 6 months but not very optimistic.

1. has anyone had results only start after 6 months?

2. what do i do if i get to a year and no change? what next? rogaine doesnt seem like a permanent solution and ive read it doesnt work on all of top of scalp.

3. anything i should add to regime that works to slow down level of loss everyday?

i've had no sides on finasteride at all. use keto shampoo once a week.

need help asap

Dutasteride, RU, CB or pray that you can maintain on minoxidil before a better treatment comes out.

The hairs you lose, do they disproportionately come from areas that experience balding (the top of the head) or are they from all over?

How do you get it?

You don't actually need to buy it online like most people on this forum do. What I did was to send a bunch of emails to several private clinics in my area asking if they are willing to prescribe dutasteride. This way you wont need to pay 50 dollars to get an appointment just to get declined. Dutasteride is alot harder to get than finasteride. Skin clinics is your best bet if from what I have experience and heard from other people. You need to go on the private market, cause regular doctor will probably not prescribe it off label.

Getting it from a doctor and being coached by a professional is alot better either way, so you know what you get and you get it legally.

Yeah, if you want antigonadotropic effects (reduction in testosterone and balls shrinking), and potential feminized/androgynous look.
What people don't seem to understand is that if you take estradiol orally, you need to take above threshold levels for it to do anything on your hairline and it already has systemic effects in your whole body. And endogenous hormones are a replacement therapy. You add 0.5mg estradiol, and you natural estradiol will be ereased cause estradiol has antigonadotropic effects and shuts down your hypothalamus in the brain. So you get less testosterone being produced and less natural estradiol being produced from aromatization from testosterone.

Estradiol is also half bad and half good for you hair, so you have lots of better compund out there which are better. Like estradiol-beta selective agonist, which is basically just the only receptor you need to activate and you will get almost all the benefits of estradiol without getting feminization. The estradiol-alpha which gives you softer skin and gyno is actually bad receptor activation for your hairline, and the only reason its halfway good is because it reduces your testosterone which is a weaker androgen (with some beta activation). That effects is not as powerful as many people seem to think, cause DHT has markedly better affinity for the receptor at your scalp.

The only problem is sourcing for powerful beta selective agonist with 100-1000 fold ratio over the alpha type. They do not exist on open market as far as I know and they are very hard to dose yourself. S-equol has better affinity for the beta, but its not that selective and much less potent.

And you want the ones with high beta to alpha ratio and as little effect on other types of receptors as possible. Cause if you study estrogens, different estrogens have different effects in different body parts, just like DHT compared to testosterone. Very hard scientific stuff which are not fully understood. My guess is that there may be more subtypes of the androgen receptor and estrogen receptor.

@LastHope123 any from of er activation will give you sides still. Not wise to take them orally

I find little research on that, probably because the beta receptor were "found" 10-20 years ago or something like that.
But from what I've read it does not activate gene transcription in breast tissue at least (actually anti-estrogenic in breast tissue like tamixofen), but it has effects in uterus, prostate and some parts of the brain. I don't know if all estrogen-beta receptor selective agonists are antigonadotropic, cause I found one that did not have any notable (so here's the problem) effect on gonadotropin release - I just don't remember the name of the compound. So if you have a compund that does not have the estrogenic effect in the breast tissue and no effect on gonadotropin release, you basically have a estrogen with next to no side effects in a male. But as long as there are no studies on estrogen-beta and hairloss, who knows if it will have a good effect on the hairline or not.

Hard to say if dutasteride is helping, cause I have only been on it for almost 2 months. I think I had a bad shed in the beginning, and new vellus hairs have started to grow about 1-2 cm beneath the hairline in the front and temples. They seem a bit stronger than what I experienced on finasteride, so I hope they turn into terminals.

But I am currently thinking about buzzing my hair next time I'm cutting my hair, maybe like 5mm to 1cm. And if I think my headshape fits the buzzed hair I'm thinking I'll just man up and let the hair slowly go nature's way. I will probably still use dutasteride and minoxidil if I want to grow it out again though, but I'll be more ready mentally if I get used to next to bald head. I still have a nw2-nw2.5, even after use of anabolic steroids. So I don't think my hairloss is so aggressive that I will go norwood 4 before turning 30 at least. My father at 60 years old is nw5a and my grandfather (father of my mother) is nw3 with diffuse thinning only, at 75.

I'm too scared to do anything with testosterone since I basically need it physically and mentally. I cannot compete physcially without testosterone. Sure, I may loose a bit of game if I go bald (if I don't rock it, who knows), but almost turning into a female without confidence will guarantee a complete loss in the heterosexual department.
