Hi.
I've been taking dutasteride (4 tablets) DAILY for the past 2 years. Was also on a lower dose (.5mg) for a couple years before that. I upped the dose (after endocrinological consultation) after finding out my serum DHT was a whopping 1090pg/ml, even after years of taking .5mg a day. All this ofcourse to combat the really aggressive hairloss I was cursed to endure since age 20. A week ago, I had another serum DHT test done and guess what, STILL HIGH (950pg/ml), and this after 2 friggin years of 4 TABLETS PER DAY. Is my body screwed or what? I cant ever seem to make any hair gains, nor can I shake off the mini acne pores that never go away. I even have the damn things on my scalp -_-. What the hell is the meaning of this. I thought dutasteride was meant to suppress an impressive 90% of DHT, yet it didnt even make an appreciable dent in its levels.
help me :( i need answers

Go for RU or a topical anti androgen.

You attack the DHT from the T -> DHT conversion angle by blocking the 5alpha-reductase, meanwhile there are plenty of more pathways which synthesize Dihydrotestosterone.

T -> DHT
DHEA -> KT-Androstanedione -> DHT
DHEA -> Testosterone -> DHT
Pregnenolone -> ... -> DHT

I sadly can't add pictures due to a server problem of the forum, hope it'll be fixed.

The best you can do is by attacking the androgen receptor protein, so no androgens can let transcription happen of androgenic informations.

Can you elaborate please? What kind of AR antagonist is the best option here?
You could use RU58841 for topical use or orally Bicalutamide tablets, it us up to you, what you choose. I won't go further into the topic; as I am not a professional to hand out advices and not allowed to give biased statements etc. Please do your own research.

If 2mg of dutasteride isn't putting a dent into it, you're going to have to address it in different ways, and the treatments will have to be powerful. As I tell everyone, oral minoxidil has the highest success rate of improving hair because it addresses the pathway most directly (the reason the follicle dies is that eventually blood flow is cut off) and in a powerful manner. Other options would be RU58841/CB 03-01/fluridil. However, if you are sensitive to antiandrogenic medication in general, you may derive even more benefit from topical estrogen. And like PN1 said, you may need to consider bicalutamide.

How do we get it? Oral minoxidil

Very hard to get a hold of RU58841 or CB03-01 where I live. And fluridil is only sold in czech/slovakia sadly. Too expensive to import it for me. Only bicalutamide is available. Isnt that a prostate cancer treatment though? How does the dosing look like? Are there any studies that indicate its effectiveness for androgenic alopecia?

Yes but all those pathways still require that a molecule be 5 alpha reduced at some point in the chain so it doesn’t really explain why his DHT hasn’t been nuked into oblivion.

The fact that shin seems to imply that his hairloss hasn’t progressed suggests to me that this is just a case of sh*t lab work.

Probably. At best he sould do another test. On the internet you can search a lot of things about "DHT mutants".

Will bicalutamide turn me into a tranny? Is it possible to adjust the dose so that that doesnt happen? Where the hell are the topical androgen receptor blockers at???? >.>

IIRC you need to take it with an aromatase inhibitor to prevent gyno.... but that sort of negates the potential for stimulating regrowth via estrogen
