There was actually a case many years ago in JAMA of a man that did just that. Surgically castrated himself. Although he was psychologically disturbed, his intellect was of immense magnitude. The case came to public attention when he showed up in an emergency room with his abdomen partially sutured. He was still demonically plagued by sexual urges (his reason for previous self-castration) so he attempted to remove the adrenal glands. However, despite giving himself oral sedation and narcotic analgesics, he had too much pain upon trying to retract the liver and gave up, closed the field and somehow made it to the hospital. He studied surgical techniques in medical texts and acquired the instruments to do this. What puzzled the doctors beyond their amazement at the scope of his genius and how he was able to operate on himself with the expertise of a surgeon, was how he could do it with the level of anesthetics and pain killers in his system. The psychologists and anesthesiologists concluded that he was able to dissociate the motor function of his brain and awareness from the body being operated on. He performed these tasks with direct vision and a mirror positioned above him on an operating table he had also purchased. This was such a bizarre story one of our profs told us that my friend and I actually looked it up in the med school library. It may have been in New England Journal of Medicine not JAMA, so long ago to remember, but I'll never forget that one.
