I'm already on oral minoxidil and I think it helped thicken it up, but the shedding is still very present. I'm on usually 2mg a day
Up it to 5 mg. Shedding indicates either continued androgenic alopecia or Telogen Effluvium, if it’s not scarring alopecia. How long have you been shedding?

Been shedding 8 months. In the beginning it was about 150-200 hairs a day, and now it's 80 or so. I have no idea how my hair is still holding up after all this.

My scalp is also very itchy at all times, and there is noticable miniturization in 50% of the hairs I shed. My hair also was given a curly texture. And a condition called "Pili Torti" is present.

So I either have LPP (Lichen Planopilaris), or a diffuse androgenetic alopecia. If it's the latter, I'd understand why it's diffuse. Because I am very low T

Telogen Effluvium also has the effect of making hairs miniaturized. That could explain the effect on your hair. As for the curly texture and the pili torti, some people’s hair texture changes after accutane. Doesn’t necessarily mean hair loss but it could have an association.

If LPP was your diagnosis then you probably have signs of it. Did you get a biopsy to confirm?

I agree, you don’t have androgenic alopecia. At least nothing visible from your pics. Your hairline is upside down U shaped, meaning it’s not even Norwood 1 yet.

Yeah I have a very juvenile hairline, so if it is androgenetic them it would be diffuse.

The LPP was not biopsy, it was strictly the derm looking at my hair with a microscope. She noted lots of reddness

Redness could mean anything tbh. It means there’s some active inflammation. I’m surprised your meds didn’t reduce it though. Did you follow up with your derm to see if your inflammation improved? You might need a different treatment if those weren’t effective

She prescribed doxycycline. I took the initiative and ordered my own very strong meds for this disease, which were Otezla and Tofacitinib. I've been in these two medicaitons for quite some time now and I don't think they worked. So I'm not sure what the problem is. I'm thinking of going back onto the Otezla and Tofacitinib

She prescribed doxycycline. I took the initiative and ordered my own very strong meds for this disease, which were Otezla and Tofacitinib. I've been in these two medicaitons for quite some time now and I don't think they worked. So I'm not sure what the problem is. I'm thinking of going back onto the Otezla and Tofacitinib
You shouldn’t be doing that on your own. Those drugs aren’t healthy. I’m wondering why you’re even worried about finasteride if you used those drugs on your own. Besides, just from browsing those JAK inhibitors and the effect it has on LPP, you need more than a few months. Some don’t show improvement in hair growth for 9 months. I hope you tell your doctors at least

2mg oral Minoxidil is to low!
Take a minimum of 5mg per day + microneedling 1.5mm once per week.
Additional try to take 1,25mg of Finasterid per day.
Have regrowed a lot of hair with this regimen and I m 53 years old and went from a diffuse norwood 4 to a dense norwood 3, maybe 2.5 now

you are 53 years old and you are still mentally affected by baldness?

How old were you when you had it? How did you live all these years mentally with this baldness? I have had baldness since I was 19 years old and it would be interesting to hear someone's testimony.

Try using minoxidil if you’re not willing to go with finasteride. Minoxidil can slow or stop shedding
just keep watching your hair. dont use minoxidil as a full head, idk what could happen lol. i think ive heard of it kicistarting hair loss

I can just agree here.

Minoxidil can make things go worse. I would start off with androgen blockage on scalp to stop further loss, if your alopecia is androgen driven.

That’s impossible lol. Minoxidil would just thicken your hair, and revert it back to where you would have been if you stop it. Besides, he’s saying he sheds a lot plus he has diffuse thinning. I’m not sure why he showed us a pic of his hairline.

I was diagnosed with LPP recently following a biopsy. My derm said a biopsy was the only way to confirm. I had horrible itching/burning sensations. Those have gone away thanks to topical steroids and lymecycline (antiobiotic). Also taking tumeric and black pepper supplement which is a natural anti-inflammatory.

I'd highly recommend getting a biopsy done of the inflammed area - ideally this should be by a doctor that specialises in hair loss. This way you know for sure whether it's LPP/Frontal Fibrosing. My derm said a biopsy was necessary to confirm LPP/FFA - and rule out other conditions like seb derm. It's possible to have both male pattern baldness ond LPP - I believe that is actually pretty common to have both.

Also, have you tried topical steroids - this is normally the first drug to treat LPP/FFA I believe as it's considered very safe, calms the immune system and relieves itching (which for me was psychologically traumatic as I couldn't stop thinking about my scalp).

The other drugs you mentioned I believe are normally used once 'safer' drugs have been tried like topical steroids, antibiotics, hydrocholorquine etc. Don't start experimenting with drugs you haven't been prescribed - they can potentially do you more harm than good.

Also, you might want to look into hair systems if your hair loss is affecting you a lot (as it does for a lot of people dealing with hair loss). The hair systems these days are much better than they used to be - and they can look really good - you tube has some good examples of this and there is a forum on this site dealing with them. I haven't tried one myself yet but may need one because of LPP some day - I feel better just knowing they are an option at least.

Pili torti is caused by inflammatory disorders of the scalp (fibrosing pattern alopecia, LPP, lupus,etc). This results in dermal fibrosis which then leads to abnormal rotational forces on the hair follicle causing twisted appearing hair shafts. Not sure how old you are but might consider thyroid testing if not already done as hypothyroid a known cause of low T, hair shedding, and even hair miniaturization (not just DHT)

The only thing I almost left the university was work. In my last year I found a very high paying job. I really liked her and I spent a lot of time there. I started applying to essay writing services to graduate from university and have a higher education. Now I'm really very happy because I decided to move to another state and work at another job! Don't drop out of university, it would be a big mistake!
