If a guy is still losing hair at age 21 on finasteride, can he realistically expect to stabilise it enough for the next 40 years to make a hair transplant viable? What happens in 10 years when he takes a 9 month break from finasteride to try for a baby with his spouse or in 10 years when finasteride stops working or gives sides? Seems like hair transplant+finasteride combo is gambling that the guy will be able to maintain for life. Also with the diffuse thinning, a lot of doctors will refuse to put grafts among existing hair. You would need to potentially pay more for a skilled surgeon who would do that.

I am starting to think that options are very limited for aggressive bald guys.
Thanks man for demolishing what little hope i had

It won't work for life but delay it and buy time. A lot of guys would take the deal if it kept their hair (assuming they start early) for 10 years.

Don't listen to him. I've seen what he looked like and I doubt his hair loss is his biggest problem. He is 5'6 but his body frame and head shape is 10x better than ours. If he was 6'0 he would have been getting dates regularly because he had the incredible frame to go with it. There is a niche of women that look for tall and strong. Like "you mess with me you mess with my bf who can kick your ***".

Hair loss will progress whether you give up or don't. The more you fight it the more it will break you when it wins. The only way out is to shave the head and accept the lower quality of life or go with hair transplant + finasteride with the caveats I described. hair transplant without finasteride is only workable when all of the hair has been lost and you see your terminal pattern, or your doctor can be sure of it. It will be a long ride to get there. hair transplant with finasteride is workable but needs you to stay on finasteride for life.

Then start finasteride again. Minoxidil will not save your hair for long. Maybe 2 years, 3 or more if you are lucky.
This is true. Do not expect to be above baseline after 2 years with minoxidil alone. Don't stop using it for sure but understand that it definitely doesn't work for long.

Ever watch the bald truth podcast? Spencer kobren kept most of his hair for 25 years on propecia. At a certain age baldness is almost irrelevant as women age 21 to 30 will not go for men who are too old anyway but women 31 to 40 will compromise as long as you have other qualities.

minoxidil alone isn't even fighting. It is just a mental fight at this stage. It is taking up mental energy that could be used elsewhere. 10 years from now you will be 30-32 and full on NW7 and living your life with whatever that entails. We have to assume the worst here. It might not be a great life but it will be a life. Now, instead of worrying these next 10 years how about living like you are the 30-32 year old self with the NW7?

In the first year of use minoxidil can work as a preventative. Example you start min and after a year you add finasteride. It might be better than if you waited a year and started finasteride because min can maintain for a short time(a year or two). But that is under the condition you respond to min, it has like 50% maintenance rate and 30% regrowth rate if im not mistaken.

Yes but I said min alone. When you combine it with finasteride, things like that are possible for years. But as far as I recall, people who used min alone were below baseline after 5 years. For an aggresive balder in the 20s, I cut that to 2 years because that's more in line with my experience using min alone.

Min users are worse than placebo after 2 years, meaning that doing nothing is better than using min for longer than 2 years.
