It is critical to distinguish between a drug losing all efficacy and a drug peaking and stabilising.
According to top-line data from Kintor's year-long study, patients using 0.5% pyrilutamide twice daily still experienced significant increases in target area hair count (TAHC) and hair width (TAHW) at 52 weeks compared to where they started (baseline). https://en.kintor.com.cn/news_details/9.html

This data looks impressive at first glance, but in reality, it's not as good as min or finasteride.
Unlike oral finasteride or dutasteride, which reduce systemic DHT levels, pyrilutamide is designed to work locally with a different mechanism of action, so is a good option for those that can't or won't take finasteride or dutasteride.
Pyrilutamide 1.0% BID (twice daily): +15.33 hairs/cm² from baseline, which was +10.65 hairs/cm² over placebo. Pyrilutamide 0.5% BID (twice daily): +14.46 hairs/cm² from baseline, which was +9.78 hairs/cm² over placebo.
The placebo-adjusted gain looks competitive with established therapies. However, cross-trial comparisons should be made cautiously: populations, endpoints, baseline severity, counting methods, and timepoints differ across trials.
An open-label clinical observational trial found that combining pyrilutamide (0.5% BID) with minoxidil (5% BID) is superior to minoxidil monotherapy. The combination group showed a mean target area non-vellus hair count (TAHC) increase of +30.54 hairs/cm² from baseline. The combination exceeded minoxidil monotherapy by 10.29 hairs/cm², a statistically significant difference. A higher proportion of patients in the combination group achieved improvements of ≥ 20 and ≥ 40 hairs/cm², alongside numerical improvements in hair growth assessment (HGA) scores. Both groups showed comparable, favourable safety profiles and good tolerability.
