The majority of people are in the middle, with only very few at the extremes. That's why you very rarely call people a '1' or a '10 - 3s and 7s are much more common.

Therefore, it does not follow from this guy being in the bottom 20% that he's a 1 or a 2. He's probably, like you say, a 3 or a 4.

I should add that if I were in this guy's position, I would get a rug. I think this guy's someone who'd look much, much better with hair, and he's not really a hair transplant candidate.
