This is in my opinion why hair transplants can be a bit of a trap. At the end of it all everything still boils down to that one drug finasteride and the hope that it will work long term without negatively affecting health.

This. If finasteride fails you are basically in trouble and at the mercy of genetics. Hopefully the final pattern is at worst a norwood 5a.

Have you tried topical finasteride
Apparently people have reported less sides from it
is it hair miniaturisation or receding or mature hairline ?

Possibly, it is hard to tell as maturing hairlines look like that also as they transition. You need to see a dermatologist for a diagnosis either way.

Is there any balding in your family?

That is good news. Keep on using it then and if you notice deterioration at any point see a dermatologist to discuss other options.

As a veteran finasteride and minoxidil user, generally speaking I advocate for people to try it, but there is a certain profile that I would recommend it for:

a) diffuse thinners as they respond best
b) significant enough hair loss to warrant even trying it, not Norwood 1 guys who want their hairline when they were 12 years old.

I haven't seen pictures of the top of your hair but would be happy to give my opinion if you provide them. Also, I'm assuming you workout, are generally healthy, as this helps reduce the chance of side effects in my opinion.

Saw your pictures, I think finasteride can definitely help and likely will help. It looks like you also have more grafts available for another FUE as your donor looks good, which finasteride will help keep strong.

You will probably also get decent regrowth from minoxidil, but I understand that it is a hassle (it is for me and practically everyone). But it is a trade off for more hair, it is what it is.

no such thing as a mature hairline

a mature hairline is at best case just very slow recession
A person's final hair loss pattern may be NW2. In that case, we might call their NW2 a mature hairline because the hairline isn't actively changing. Look at Roger Federer and Harry Styles. Beautiful mature hairlines.

Federer has nice hair but he is a NW3

Again, who knows, if it will get worse as he gets older - one thing for certain, hairloss is hairloss

if you are losing hair you are prone to male pattern baldness.

if I could go back to my 20s, I would tell myself to see a derm and start treating it like I am doing now

Harry Styles is definitely on finasteride, there were reports that he panicked/started crying when his hair loss started. But he has kept the same hairline since then.

Federer has had that hairline for over 10 years. He is not actively losing hair and so that's a mature hairline in my view (assuming that he is not on meds). But we are just arguing semantics here.

I used minoxidil religiously for 5 years. I didn't miss a day. Christmas, birthdays, vacations, road trips, airpline rides, weddings and events.... didn't do a thing for me. So you should count yourself lucky that you found a treatment that you are satisfied with and which works for you.

sorry to hear that, sounds like your hairloss is very aggressive

at least you treated it

are lots of people in your family bald?

He should have done more research to realize that for aggressive hair loss, minoxidil by itself is pointless without finasteride.
